• Position: RN Patient Navigator - Neurosciences
• Location: Arlington Heights IL
• Full Time 40 hours/week
• Hours: 8am-4:30pm Monday - Friday
• Travel Required: No

What you will do:

Reporting to the specialty program leader for Neuroscience the Patient Navigator serves as the primary point of contact for the patient through the continuum of care; diagnosis planning treatment pre and post-operative care discharge and follow up. The Patient Navigator functions as a resource to patients seeking patient input into the plan of care. The Patient Navigator engages and collaborates with all appropriate care givers (physicians and the entire care team) in the plan of care while serving as the advocate for the patient. The Patient Navigator provides patient and family education and access to hospital and community resources as well as community education and screening.

• Facilitates the entry of patients into the program and coordinates their care.
• Conduct comprehensive assessments of patients medical social and psychological needs.
• Collaborate with healthcare providers to develop individualized care plans that address patients unique needs and goals.
• Identify potential barriers to care and work to address them proactively.
• Serve as a liaison between patients families healthcare providers and community resources to ensure a smooth transition of care.
• Monitor and manage patients progress throughout their healthcare journey.

What you will need:

• Bachelors Degree Nursing required
• Inpatient nursing for at least (1) year
• Experience working with neurology or ENT patient populations preferred
• Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) Current Registered Nurse (RN) license in the state of Illinois. required
