Core Responsibilities:
As the Children and Youth with Special Health Care Needs (CYSHCN) Care Coordinator you will be responsible for promoting the health of children and youth with special health care needs through state-wide initiatives and collaborations as well as through specific public health programs. These are children from birth to 21 years of age who have a long term chronic physical developmental behavioral or emotional illness or condition.
You will be overseeing an assistance program where you will be responsible for enrolling clients based on eligibility and ensuring financial assistance is received. You will be working directly with Maine families and their children with special health care needs to ensure they are connected to resources to meet their medical needs.
As the CYSHCN Coordinator you will work with many community partners to provide education on the Healthcare Transition process and effective care coordination for families and their children with special health care needs. A comprehensive knowledge of Title V and the Children and Youth with Special Health Care Needs Domain project management effective communication and experienced knowledge of public health principles practices and education are essential to this position.
You will also:
- Build relationships and coordinate with internal and external providers to ensure needs of the clients are met.
- Oversee the enrollment process for the assistance program and maintain client files throughout the year.
- Provide support to families and providers by providing technical assistance to parents of children and youth with special health care needs one-on-one individualized technical assistance (i.e. telephone mail and email) outreach and workshops/training.
- Provide support in completing prior authorizations for health services.
Minimum Qualifications:
An eight (8) year combination of experience and/or education in medical services to include at least three (3) years of professional experience in coordinating and administering programs for children with special medical needs or related disorders.
Preferred Skills
- Excellent oral and written communication skills.
- Demonstrated ability to engage in compassionate strength-based conversations with youth and families.
- Knowledge of maternal and child health within a public health framework.
- Knowledge of issues systems and resources related to children and youth with special health care needs.
- Ability to establish and maintain effective working relationships with diverse internal and external partners.
- Experience facilitating or guiding strategic discussions and supporting groups in decision-making aligned with program goals.
- Demonstrated experience navigating complex systems (e.g. healthcare behavioral health education and social services) and supporting families in accessing and coordinating services.
- Lived experience as an individual with special health care needs or as a family member/caregiver of a child or youth with special health care needs is highly valued and may be considered in combination with professional experience.
- Experience working in or alongside social work case management or family support roles with an emphasis on family-centered culturally responsive approaches.
