Description
We are looking for a Medical Social Worker to provide care at our Patients homes in primarily Alameda County and some Santa Clara Counties. As a Medical Social Worker you will assist patients and families in coping with problems resulting from severe or long-term illness. You will assess diagnose and treat patients mental and social conditions counsel individuals and/ or families and update case records.
Requirements
- Assesses psychosocial status of patient and social/ emotional factors related to the patients illness need for care and response to treatment.
- Assisting patients and families with goal setting counseling and planning and providing interventions.
- Assisting patients and families with appropriate community recourses their eligibility criteria and regulations.
- Participates in the development and periodic renewal of the patients plan of care.
- Observes and reports to the physician the response to the patients treatment.
- Maintains verbal and written communication with other personnel involved with the care of the patient.
- Maintains appropriate and timely patient care records in the EMR including observation treatments patient response to treatment and change in patients condition
- Attends and participates in appropriate patients care conferences staff meetings and agency committee meetings.
- Ability to establish and maintain effective working relationships with patients and families and staff members.
Salary Description
$45 - $50 an hour
