Key Responsibilities:

• Perform physical therapy assessments in the clinic and occasionally in participants homes to identify care needs and personal goals
  
• Partner with the interdisciplinary team to design and update tailored care plans
  
• Deliver and document therapy interventions including exercise mobility and functional training for activities of daily living
  
• Apply therapeutic techniques such as massage heat hydrotherapy or electrotherapy when appropriate
  
• Provide guidance to therapy assistants caregivers and families to ensure effective continuation of therapy and safe use of equipment
  

Requirements

• Bachelors degree in Physical Therapy required; masters or doctoral degree preferred
  
• Two (2) years of experience in a clinical setting working with older or medically complex populations preferred
  
• Current California Physical Therapy license or license eligibility
  
• CPR and First Aid certification required (or must be obtained within 180 days of hire)
