We are seeking an experienced and compassionate Physical Therapist to join a collaborative interdisciplinary team dedicated to delivering exceptional person-centered care for older this role youll focus on helping participants maintain independence and improve their quality of life through individualized therapy and wellness programs.
Unlike traditional therapy settings our Physical Therapists enjoy the ability to truly connect with participants and make a lasting differencewithout the high-volume pressures of typical outpatient or inpatient environments.

- Manage a lower caseload and dedicate time to each participants progress
- Build long-term therapeutic relationships not short-term rehab interactions
- Collaborate daily with a diverse team of healthcare professionals

Key Responsibilities:
- Perform physical therapy assessments in the clinic and occasionally in participants homes to identify care needs and personal goals
- Partner with the interdisciplinary team to design and update tailored care plans
- Deliver and document therapy interventions including exercise mobility and functional training for activities of daily living
- Apply therapeutic techniques such as massage heat hydrotherapy or electrotherapy when appropriate
- Provide guidance to therapy assistants caregivers and families to ensure effective continuation of therapy and safe use of equipment
Requirements
- Bachelors degree in Physical Therapy required; masters or doctoral degree preferred
- Two (2) years of experience in a clinical setting working with older or medically complex populations preferred
- Current California Physical Therapy license or license eligibility
- CPR and First Aid certification required (or must be obtained within 180 days of hire)
