Key Responsibilities
Assessment: Conduct thorough initial evaluations of patient mobility strength and functional limitations.
Plan of Care: Develop and implement individualized 14-week treatment plans with measurable goals.
Therapy Delivery: Administer manual therapy therapeutic exercises and gait training to improve patient outcomes.
Collaboration: Work closely with the Chapel Manor interdisciplinary team (OTs SLPs and Nursing) to coordinate comprehensive care.
Documentation: Maintain timely and accurate clinical records in compliance with facility and state standards.
Education: Instruct patients and families on safe mobility techniques and home exercise programs.
Requirements
Education: Graduate of an accredited Physical Therapy program (Masters or Doctorate).
Licensure: Active Pennsylvania Physical Therapist license.
Certifications: Current CPR/BLS certification.
Expertise: Strong clinical reasoning and proficiency in geriatric rehabilitation.
Interpersonal Skills: Excellent communication skills to motivate patients and collaborate with staff.
