Physical Homecare Therapist

Position Summary:
A Home Health Physical Therapist provides skilled rehabilitation services in patients homes to improve mobility strength balance and functional independence. This role focuses on delivering individualized patient-centered care in a home setting to support recovery prevent decline and enhance quality of life.

Key Responsibilities:

• Perform in-home physical therapy evaluations and functional assessments
• Develop and implement individualized treatment plans based on patient needs and home environment
• Provide skilled interventions including therapeutic exercise gait training balance training and functional mobility training
• Assess home safety risks and recommend adaptive equipment or environmental modifications
• Educate patients and caregivers on safe mobility fall prevention and home exercise programs
• Monitor patient progress and adjust treatment plans as needed
• Document patient care accurately and in a timely manner according to agency and regulatory standards
• Collaborate with physicians nurses and other members of the healthcare team
• Travel to patient homes within assigned geographic area

Qualifications:

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
• Current state Physical Therapist licensure
• CPR certification (or ability to obtain)
• Valid drivers license and reliable transportation
• Prior home health or outpatient experience preferred

Work Environment:

• Patient homes within assigned service area
• Regular travel required
• Flexible schedule based on caseload and patient needs

Physical Requirements:

• Ability to assist patients with transfers ambulation and mobility tasks
• Frequent standing walking bending and light-to-moderate physical activity
• Ability to safely work in non-clinical environments
