We are looking for a Case Manager Housing Locator & Mental Health Professional (MHP) Street Outreachwho demonstrates this mission and wants to work for an organization that makes a difference in the community.
Schedule: Monday-Friday 9am-5m (Flexible)
What will you be doing
The Street Outreach Case Manager is responsible for contacting individuals who are inappropriately using Crisis Services through Police and Fire identified by FUSE committees or located in tent cities and other public spaces and attempting to engage them in more clinically appropriate services and access shelter/housing. The case manager will provide outreach and case management advocacy evidence based mental health and support services to individuals with mental health diagnosis and provide appropriate and warm hand offs to referrals. Case Managers will engage clients in the community where they are located and will assist in clients identifying natural supports. Hours include evenings and weekends.
Essential Job Responsibilities:
Attend to scheduled work hours and be flexible to meet client and program needs as assigned by Program Manager or Director.
Respond to outreach calls from shift start to shift end.
Respond to calls upon being dispatched
Complete assigned office hours.
Attend soup kitchens breakfasts and community dinners
Complete all mandatory and additional trainings
Provide advocacy and support to individuals experiencing mental health concerns
Complete clinical assessments that meet the requirements of program funders and applicable administrative rules
Provide case management services as medically necessary
Complete required documentation (assessments treatment plans case notes VISPDATs consents monthly case checks ROI) on a timely basis
Provide appropriate follow-ups to establish rapport with individual.
Acquire/maintain certification in First Aid CPR CPI Mandated Reporting and other agency required trainings.
Abide by AIDs Code of Ethics policies and procedures
Maintain professional judgment and performance regardless of personal issues
Contact people inappropriately using Crisis Services and attempt to engage them in more clinically appropriate services.
Complete and maintain monthly and quarterly assigned reports.
Maintain and update Outlook calendar with daily details of activities.
Input information into HMIS.
Participate in community meetings.
Obtain input from consumers and other stakeholders on how to improve service provision
Provide outcome evaluation data to supervisor
Assure client linkage to community resource.
Maintain a positive professional working relationship with co-workers agencies and other stakeholders
Promote and maintain client rights assure client choice
Assist clients in maintaining public entitlements.
Participate in outreach events (i.e. resource fairs/events) to promote program services and expand network.
Remain informed about AID services in order to provide appropriate referrals.
Participate in agency committees as assigned
Provide outreach services to clients who are unable or unwilling to engage in office-based services

Qualifications
Qualifications:
Education: Bachelors degree in human services field required Masters Degree preferred
License or Certification: ASSIST and SAMHSA SOAR certification preferred.
Experience:
Experience with at-risk clientele and crisis work. At least 1 year of experience in behavioral health and case management preferred
Bilingual preferred
Physical:
Position requires ability to communicate in-person telephone and in group settings
Requires frequent standing; walking navigation of stairs on a regular basis Ability to provide services in clients homes and in the community
No lifting restrictions and Navigation of stairs
Equipment:
Position operates in a professional office environment and off-site in various community settings. Role routinely uses standard office equipment such as computers phones copiers and filing cabinets
Additional Requirements:
Evening weekends
Valid Illinois drivers license
Drive self and clients in personal vehicle
Must have the minimal amount of liability insurance on personal automobile as defined by AID handbook
