We are seeking a skilled and compassionate Physical Therapist (PT) to join a reputable Critical Access Hospital providing adult outpatient rehabilitation services in Peridot Arizona. In this role you will evaluate and treat a diverse adult patient population developing individualized evidence-based treatment plans to address orthopedic post-surgical neurological and mobility-related conditions. As part of a critical access facility you will have the opportunity to work with a wide range of diagnoses and functional needs helping patients improve strength balance mobility and overall function so they can return safely to their daily activities.
You will collaborate closely with physicians nursing staff and other healthcare professionals to ensure continuity of care and measurable patient progress. Responsibilities
Apply strong clinical reasoning to evaluate adult patients determine diagnoses and develop individualized plans of care based on physician referrals and clinical findings
Demonstrate clinical competencies across a diverse adult population adjusting treatment techniques to address orthopedic injuries post-surgical recovery neurological conditions chronic pain and mobility limitations
Implement evidence-based therapeutic interventions and modify treatment plans as needed based on patient response and measurable progress
Maintain accurate and timely documentation of evaluations treatment sessions progress notes and discharge summaries in compliance with state regulations hospital policies and clinic standards
Collaborate closely with patients physicians and interdisciplinary healthcare team members to support continuity of care and promote optimal functional outcomes
Requirements
Active state license as a Physical Therapist
Current BLS/CPR certification
Minimum of 6 months of relevant PT experience within the last 5 years
