Job Title/Position: Director of Nursing
Location: Oregon (Portland Metro)
Reports To: Executive Director
Position Summary
The Director of Nursing (DON) provides leadership and oversight for all hospice clinical operations. This role is responsible for ensuring high-quality patient care regulatory compliance and strong team performance while balancing operational and financial outcomes. This is a hands-on leadership role requiring both clinical expertise and the ability to lead develop and hold teams accountable in a fast-paced hospice environment.
Key Responsibilities
Clinical & Operational Leadership
- Lead day-to-day clinical operations including patient assignments productivity and census management
- Provide guidance and support in complex patient care situations
- Monitor key metrics such as census acuity visit utilization and staffing needs
- Ensure alignment between clinical operations and organizational goals
- Maintain visibility with field staff and support teams as needed
Staff Leadership & Accountability
- Lead hiring onboarding coaching and performance management of clinical staff
- Hold leaders and staff accountable for performance attendance and productivity
- Address performance concerns proactively including corrective action when necessary
- Develop and mentor clinical supervisors and emerging leaders
- Foster a culture of accountability teamwork and continuous improvement
Quality Compliance & Clinical Excellence
- Ensure all clinical documentation is accurate timely and audit-ready
- Maintain compliance with Medicare Conditions of Participation and regulatory standards
- Lead QAPI initiatives including quality metrics incidents and infection control
- Prepare for surveys audits and accreditation reviews
- Oversee incident investigations and corrective actions
Operational & Financial Management
- Monitor staffing utilization overtime and productivity expectations
- Partner with leadership on clinical budget planning
- Ensure appropriate use of supplies equipment and medications
Communication & Community Engagement
- Promote clear professional communication across teams
- Build relationships with referral sources including hospitals and SNFs
- Collaborate cross-functionally to ensure continuity of care
Emergency Management
- Ensure staff safety and coordination during emergencies
- Lead communication with internal teams and external agencies
- Maintain infection control and safety standards during emergencies
Role Expectations (Non-Negotiables)
- This is not a desk-only roleactive involvement in clinical operations is expected
- Must be comfortable making independent decisions related to staffing and patient care
- Accountable for team performance documentation quality and regulatory outcomes
- Must be able to lead in a dynamic environment with changing census and patient needs
Qualifications
- Active Oregon RN license in good standing
- 7 years of clinical experience
- 3 years in a leadership role (hospice preferred)
- Experience in hospice home health SNF or hospital settings
- Strong leadership and communication skills
- Knowledge of regulatory standards and compliance requirements
- Valid drivers license and reliable transportation
