Job Summary
Lead Care Manager (ECM) Hybrid Role

Join Heritage Health Network and make a meaningful impact in your community as a Lead Care Manager supporting members through our Enhanced Care Management (ECM) program.
This is a hybrid position (2 days remote 3 days in the field) giving you the flexibility to work from home while building strong in-person connections with members across Los Angeles County.
Key Responsibilities
- Serve as the primary point of contact for members and coordinate their care
- Develop and manage personalized care plans with members and providers
- Collaborate with healthcare teams and community resources
- Support members through education advocacy and engagement
- Identify and address gaps in care
- Maintain accurate timely documentation
Requirements
- Experience in care or case management (community-based preferred)
- Knowledge of Medi-Cal and ECM strongly preferred
- Strong communication and interpersonal skills
- Ability to work in a team-oriented environment
- Willingness to travel within Los Angeles County
- Bilingual (English/Spanish) preferred

Required Education:
