Bilingual Community Health Worker Inland Empire & Desert Regions (Travel Role)
Heritage Health Network (HHN)
Join Heritage Health Network as a Bilingual Community Health Worker (CHW) and help connect Medi-Cal members to care through our Enhanced Care Management (ECM) program.
This hybrid role requires 3 days in the field per week at community locations (food banks shelters clinics churches probation offices hospitals) and 23 days conducting telehealth outreach onsite in Riverside CA. Spanish fluency is required.
Key Responsibilities
- Conduct telehealth outreach to members and screen for ECM eligibility
- Submit referrals to health plans and document interactions
- Engage in field outreach to identify and support potential ECM participants
- Build relationships with community partners and assist members in accessing healthcare and social resources
Requirements
- High school diploma required; BA or Masters in Public Health Social Work or related field preferred
- Experience in community outreach social services or healthcare navigation preferred
- Valid drivers license active auto insurance and reliable vehicle for field work
- Bilingual in Spanish and English
- Strong communication organizational and time management skills
