We are seeking an experienced Behavioral Health Specialist to provide outpatient behavioral health services in a collaborative integrated care environment. This role is ideal for an LPC or LMSW who is passionate about helping patients improve their mental health while working closely with primary care providers and interdisciplinary teams.
This is a structured outpatient-only position with a predictable schedule and the opportunity to build meaningful relationships with patients through ongoing therapy and support.
Responsibilities
Conduct comprehensive mental health assessments and develop individualized treatment plans
Provide outpatient therapy to adults and/or children
Deliver crisis intervention and behavioral stabilization as needed
Maintain a structured caseload to ensure quality patient-centered care
Collaborate with primary care providers care managers and interdisciplinary teams
Connect patients with community and behavioral health resources
Maintain accurate treatment plans progress notes and clinical documentation
Monitor patient outcomes and adjust interventions as needed
Prepare discharge plans and case summaries
Requirements
Qualifications
Masters Degree in Social Work Counseling or a related field
Active LPC or LMSW license required
Minimum of 3 years of outpatient behavioral health experience
Experience with mental health assessments treatment planning therapy and crisis intervention
Strong communication organization and collaboration skills
Comfortable working in a structured team-based outpatient environment
Passion for serving vulnerable populations and improving patient outcomes
