Your Role: Home Health Physical Therapist

As aHome Health Physical Therapist (PT) you will deliverskilled physical therapy services in patients homes assisted living facilities and senior communities throughoutLos Angeles and surrounding areas.

Your work will focus onrehabilitation fall prevention mobility improvement and restoring independencefor patients recovering from injury illness surgery or chronic conditions includingdementia and geriatric health challenges.

Key Responsibilities

Conductin-home physical therapy evaluations and follow-up visits
Developindividualized rehabilitation treatment plans
Improvestrength balance mobility and functional independence
Provide therapy togeriatric and senior patients in home-based care settings
Educate patients and caregivers onsafe movement fall prevention and assistive equipment
Document visits inWellSky KinnserEMR
Collaborate with physicians nurses and interdisciplinary care teams
Provide therapy inhomes assisted living facilities and senior communities

Qualifications

Licensed Physical Therapist (PT) California
CPR Certification (AHA approved)
Minimum1 year of clinical experience(Home Health experience preferred)
Experience working withgeriatric patients dementia care or senior populationspreferred
Strongcommunication clinical judgment and patient education skills
Validdrivers license reliable transportation and auto insurance
