We are seeking a Full-time Licensed Clinical Therapist to join ourAssertive Community Treatment Program (ACT) for our Chicago Baltimore IL location.

Hours - Monday through Friday 9AM-5PM however open to being flexible with hours as needed.

Position Summary

Under the general supervision of the Clinical Services Manager Program Director Executive Director or Designee. This position works to make a meaningful difference in the lives of the individuals that the Front Door Diversion Program and Assertive Community Treatment (ACT) program serves. This position works closely with individuals families and community partners providing therapy advocacy and support toward the ultimate goal of living safely and successfully in the community. Provides clinical support to employees within assigned department or program.

RESPONSIBILITIES

The FDDP Clinical Therapist position may include the following as well as other duties assigned:

  • Actively supports represents and extends the mission vision and values of the organization.
  • Provides appropriate individual group and family mental health services case management transportation and other professional services meeting or exceeding established outcomes thresholds on behalf of persons served.
  • Travels to provide community based mental health services to individuals in the community to remain in compliance with organizational contract and regulatory requirements.
  • Completes all required documentation in a timely manner.
  • Completes all IMCANS and treatment planning documentation in a timely manner.
  • Completes all required training in a timely manner.
  • Consistently achieves established productivity thresholds.
  • Coordinate and monitor services of individuals in the community including but not limited to mental health medical educational housing psychological vocational and social services.
  • Regularly reviews and modifies treatment plans to assure consistent progress and success on written treatment plan goals.
  • Establishes and maintains a strong relationship with individuals served and other individuals involved in their care.
  • Assists in the development of adult living skills to assist clients with maintaining their own independent living option.
  • Provides safety assessment crisis response and behavioral stabilization services as required to assure safety and stability for individuals served.
  • Provides evidenced-based service modalities in mental health service delivery.
  • Participatein the Continuous Quality Improvement activities on a monthly/quarterly basis.
  • Actively participates in the weekly supervision process.

MINIMUM QUALIFICATIONS

  • Masters degree in social work counseling or comparable Human Services field with valid state Clinical licensure (LCSW LCPC LMFT)
  • 3 years of work experience working in direct service with mental health and substance use preferred.
  • A valid professional clinical license is required and acceptable by state or contract regulations. CADC and/or CRSS preferred.
  • Must have a valid drivers license reliable transportation automotive insurance and a good driving record.
  • A willingness to work flexible and non-traditional hours in the service of individuals served.
  • Capacity to be flexible and responsive to individuals served system partners and internal customers.
  • Capacity to remain objective and professional in all areas of job function.
  • Demonstrates tolerance and respect for the ideas and actions of others.
  • Capacity to effectively work with and be respectful and sensitive to people from various cultures socioeconomic ethnic religious and racial backgrounds.
  • Consistently exercises discretion and independent judgement. Readily confers with supervisors in complex or unique situations.
  • Demonstrates strategic and critical thinking resulting in effective time and task management.
  • Demonstrated ability to coach others transferring knowledge and information in easily understandable terms.
  • Capacity to interpret and apply organizational policies and procedures.
  • Demonstrated capacity to remain objective yet connected manages reactivity and self-regulates in response to challenges.
  • Proficient use of desktop and laptop computers smart phones and tablets printers fax machines and photocopiers as well as software including word processing spreadsheet and database programs.

  • Valid drivers license

  • Minimum automobile insurance coverage of $10000/$300000 bodily injury liability
