Physical Therapist Home care

Position Summary:
A Home Health Physical Therapist provides skilled rehabilitation services to patients in their homes. The role focuses on restoring movement improving functional ability reducing pain and helping patients safely remain independent in their home environment.

Essential Responsibilities:

• Conduct in-home physical therapy evaluations and develop individualized care plans
• Treat patients recovering from surgery injury illness or managing chronic conditions
• Implement therapeutic exercises mobility training balance and gait training and functional strengthening
• Assess home safety and recommend modifications or assistive devices as needed
• Educate patients and caregivers on exercises safety techniques and injury prevention strategies
• Monitor patient progress and adjust treatment plans based on outcomes
• Maintain accurate and timely documentation in accordance with regulatory and agency standards
• Coordinate care with physicians nurses and other rehabilitation professionals
• Travel to patient homes within assigned territory

Qualifications:

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
• Current state licensure as a Physical Therapist
• Valid drivers license and reliable transportation
• CPR certification required or ability to obtain
• Home health or outpatient experience preferred
