Full time home health LPN Licensed Practical Nurse to support the Advocate Health at Home South East team. Normal office hours are Monday through Friday 8:00am to 5:00pm. Rotating holiday schedule of 2 holidays per calendar year.
Pay Range
$25.30 - $37.95LPN Home Health Southeast City

Position Overview
Join our Home Health team as a Licensed Practical Nurse (LPN) delivering compassionate high-quality care to patients in the comfort of their homes. Working under the supervision of an RN youll play a key role in supporting patient outcomes coordinating care and empowering patients and families through education.
Key Responsibilities
- Deliver skilled nursing care in accordance with individualized care plans under RN supervision
- Monitor patient conditions recognize changes and communicate updates to the care team
- Coordinate care with interdisciplinary team members to ensure continuity and optimal outcomes
- Provide patient and family education on disease management treatments medications and safety
- Accurately document care to support clinical regulatory and reimbursement requirements
- Participate in case conferences team meetings and ongoing care planning
- Promote patient independence and engagement through education and support
- Maintain compliance with organizational policies and quality/safety standards
Required Qualifications
- Active LPN license in the State of Illinois (or eligible)
- CPR Certification
- Pharmacology certification
- Minimum 2 years of Med/Surg clinical experience
- Valid drivers license and reliable transportation
Preferred Experience
- Prior home health experience
- Familiarity with Medicare/Medicaid home health guidelines
Skills & Strengths
- Strong clinical assessment and critical thinking skills
- Excellent communication and documentation abilities
- Ability to work independently and manage time effectively
- Comfortable using EMR systems and basic Microsoft Office tools
- Strong interpersonal skills with a patient-centered approach
Work Environment & Requirements
- Travel throughout assigned geographic area to patient homes
- Ability to lift up to 35 lbs (with assistance/equipment as needed)
- Frequent standing walking bending and patient assistance
- Work performed in patient homes and community settings
Compensation
$23.60 $35.30 per hour (based on experience)
