Full time home health LPN Licensed Practical Nurse to support the Advocate Health at Home South East team. Normal office hours are Monday through Friday 8:00am to 5:00pm. Rotating holiday schedule of 2 holidays per calendar year.

$25.30 - $37.95

Schedule: Full-Time MondayFriday 8:00 AM 5:00 PM

Position Overview

Join our Home Health team as a Licensed Practical Nurse (LPN) delivering compassionate high-quality care to patients in the comfort of their homes. Working under the supervision of an RN youll play a key role in supporting patient outcomes coordinating care and empowering patients and families through education.

Key Responsibilities

  • Deliver skilled nursing care in accordance with individualized care plans under RN supervision
  • Monitor patient conditions recognize changes and communicate updates to the care team
  • Coordinate care with interdisciplinary team members to ensure continuity and optimal outcomes
  • Provide patient and family education on disease management treatments medications and safety
  • Accurately document care to support clinical regulatory and reimbursement requirements
  • Participate in case conferences team meetings and ongoing care planning
  • Promote patient independence and engagement through education and support
  • Maintain compliance with organizational policies and quality/safety standards

Required Qualifications

  • Active LPN license in the State of Illinois (or eligible)
  • CPR Certification
  • Pharmacology certification
  • Minimum 2 years of Med/Surg clinical experience
  • Valid drivers license and reliable transportation

Preferred Experience

  • Prior home health experience
  • Familiarity with Medicare/Medicaid home health guidelines

  • Comfortable using EMR systems and basic Microsoft Office tools
  • Strong interpersonal skills with a patient-centered approach

Work Environment & Requirements

  • Travel throughout assigned geographic area to patient homes
  • Ability to lift up to 35 lbs (with assistance/equipment as needed)
  • Frequent standing walking bending and patient assistance
  • Work performed in patient homes and community settings

Compensation

$23.60 $35.30 per hour (based on experience)
