WHAT ARE WE OFFERING
The Barossa Hills Fleurieu Local Health Network isseekingcompassionate and motivated Community Support Workers to provide direct personalised care to aged frail or disabled individuals who wish to remain living independently in their own homes. Working under a Consumer Directed Care model you will support clients with personal care meal preparation cleaning transport and social engagement enabling them tomaintaincontrol over their care and lifestyle choices.
This role requires flexibility empathy and the ability to work autonomously while collaborating with clients their families and other health professionals. It offers a rewarding opportunity to make a meaningful difference in the lives of community members by promoting wellbeing independence and quality of life in a supportive and professional environment.

HOW WILL YOU CONTRIBUTE
You will.
- Provide personalised in-home and residential support to clients including personal care household tasks meal preparation social support andassistancewith mobility or therapy needs.
- Follow and contribute to individual care plans monitoring client wellbeing and reporting changes in health or functional status to coordinators.
- Maintain a safe clean and organised environment including equipment aids and client living spaces.
- Work collaboratively within a multi-disciplinary teamparticipatingin training quality improvement activities and client service reviews.
- Comply withworkplace health and safety standards infection control confidentiality and continuous improvement initiatives.
