Homecare PT

Job Summary:
A Homecare Physical Therapist provides skilled physical therapy services to patients in their homes helping them recover from injuries surgeries or chronic conditions while promoting independence and improving overall mobility. This role focuses on personalized patient-centered care in a home environment requiring strong clinical judgment adaptability and communication skills.

Key Responsibilities:

• Conduct comprehensive in-home evaluations of patients physical abilities functional limitations and living environments
• Develop and implement individualized treatment plans tailored to patient goals and home settings
• Provide therapeutic exercises manual therapy gait training and balance activities
• Educate patients and caregivers on safe mobility techniques fall prevention and home exercise programs
• Monitor patient progress and adjust treatment plans as needed
• Document patient care accurately and in compliance with healthcare regulations and agency policies
• Collaborate with physicians nurses occupational therapists and other healthcare professionals
• Ensure a safe and supportive home environment by recommending adaptive equipment or modifications
• Travel to patient homes within assigned service areas

Qualifications:

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
• Current state licensure as a Physical Therapist
• Valid drivers license and reliable transportation
• CPR certification (or willingness to obtain)
• Prior experience in home health or outpatient care preferred
