Job Summary
Job Overview:
A Home Health Physical Therapist delivers skilled rehabilitation services in patients homes focusing on improving mobility restoring function reducing pain and increasing independence. Care is tailored to each patients condition and living environment with an emphasis on practical real-world recovery.
Core Responsibilities:
- Perform in-home evaluations to assess physical function mobility and safety needs
- Develop and implement individualized treatment plans based on clinical findings and patient goals
- Provide therapeutic interventions such as exercise programs gait training balance training and functional mobility work
- Evaluate home environments for safety risks and recommend adaptive equipment or modifications
- Educate patients and caregivers on safe movement techniques and home exercise programs
- Track patient progress and modify treatment plans as needed
- Document all care accurately and in compliance with regulatory and agency standards
- Collaborate with physicians nurses and other members of the healthcare team
- Travel between patient homes within assigned service areas
Minimum Qualifications:
- Doctor of Physical Therapy (DPT) or equivalent degree
- Active Physical Therapist license in the practicing state
- Valid drivers license and dependable transportation
- CPR certification (or willingness to obtain)
Preferred Experience:
- Prior experience in home health outpatient or geriatric rehabilitation
