Key Responsibilities Certified Home Health Aide (CHHA)

• Providepersonal care assistanceincluding bathing grooming dressing and toileting
• Supportmobility and rehab activities including transfers ambulation and exercises
• Monitor and recordvital signs(temperature pulse respiration) during home visits
• Assist withmeal preparation and feedingbased on patient care plans
• Offermedication reminders(no administration required)
• Performlight housekeepingto maintain a safe and clean home environment
• Observe and report changes in patient condition to the clinical team
• Follow individualizedhome health care plansunder RN or therapist supervision

Qualifications CHHA (Home Health)

• ActiveCertified Home Health Aide (CHHA)certification (California)
• CPR certification(current and valid)
• Reliable transportation and ability to travel throughout Los AngelesCounty
• Strong communication skills and commitment topatient-centered care
• Home health or community care experience preferred(not required)
