Job Summary
Position Overview:
A Home Health Physical Therapist delivers rehabilitation services directly in patients homes helping individuals regain strength mobility and independence after illness injury surgery or due to chronic conditions. This role emphasizes individualized care in real-world living environments and requires strong clinical skills independence and flexibility.
Primary Duties:
- Perform in-home physical therapy evaluations and reassessments
- Develop and implement personalized treatment plans based on patient needs and home environment
- Provide hands-on therapy therapeutic exercise programs gait and balance training and functional mobility work
- Assess safety risks in the home and recommend adaptive equipment or modifications
- Educate patients and caregivers on safe movement recovery strategies and home exercise programs
- Track patient progress and adjust care plans accordingly
- Maintain timely accurate documentation in compliance with clinical and regulatory standards
- Collaborate with physicians nurses occupational therapists and other care team members
- Travel to patient residences within assigned service areas
Qualifications:
- Doctor of Physical Therapy (DPT) or equivalent degree
- Active Physical Therapist license in the state of practice
- Current CPR certification (or ability to obtain)
- Valid drivers license and dependable transportation
- Prior home health experience preferred but not required

- Ability to safely navigate various home settings
