Key Responsibilities Behavioral Health RN (Home Health)

• Perform in-home behavioral health and psychiatric nursing assessments including mental status and safety evaluations
• Deliver skilled home health nursing interventions including medication management and symptom monitoring
• Provide patient and caregiver education on mental health conditions treatment adherence and coping strategies
• Develop and implement individualized behavioral health care plans in a home and community care setting
• Coordinate care with physicians therapists and interdisciplinary teams
• Maintain accurate timely documentation in accordance with home health standards

Qualifications Behavioral Health Nurse (RN)

• Active California Registered Nurse (RN) license in good standing
• Psychiatric-Mental Health Nursing Certification (PMH-BC) is required
• Minimum 1 year of behavioral health or psychiatric nursing experience (home health or community care preferred)
• Graduate of an accredited nursing program (BSN preferred)
• Strong skills in clinical assessment crisis intervention and therapeutic communication
• Public Health Nurse (PHN) certification (preferred)
• Valid drivers license and ability to travel throughout Ventura County
