Grade 2 Transition Care Program Physiotherapist

About the Role
An opportunity currently exists for a Grade 2 physiotherapist to join the Transition Care Program. The successful applicant will be responsible for providing high quality overall physiotherapy management to patients admitted within the Transition Care Program at the St Georges Campus in Kew of St Vincents Hospital Melbourne. The role is also responsible for supervision of students Grade 1 staff and Allied Health Assistants and supporting progression of the Transition Care Program work plan.
Applications from current grade two physiotherapists or suitably experienced grade one physiotherapists will be considered. Relevant postgraduate qualifications would also be highly regarded.

Must have or be currently eligible for National Physiotherapist Registration.
Classifications for this position will range between VB1 VB4 ($90235.60 - $103724.40 per annum pro rata).
Duties include
- Appropriate timely and effective physiotherapy management of Transition Care Program clients across home-based and bed-based settings.
- Assessment planning and treatment including client goal setting in consultation with patient/family/carers as appropriate
- Work in collaboration with multidisciplinary colleagues by communicating client assessment and treatment outcomes effectively with other team members and contributing to team meetings.
- Clinical supervision and teaching of physiotherapy students both undergraduate and postgraduate as required
- Develop and promote new initiatives regarding service provision
Your Contribution
- Recent experience in and knowledge of physiotherapy management of aged care clients in a subacute health or community setting.
- Demonstrated ability to work in a multidisciplinary team
- Knowledge of and commitment to continuous quality improvement
- Experience with student supervision
- Superior written and verbal communication skills
- Home visiting experience (desirable)

Transition Care Program
The Transition Care Program which is jointly funded by the Commonwealth and State and Territories. St Vincents Transition Care Program is resourced to follow the client through the service system and avoid unnecessary extension of hospital admissions and/or premature admission to residential care.
The intent of the Transition Care Program is to provide short-term support and active management for older people at the interface of the acute/subacute and residential aged care sectors.
