Position Description: In order toovercome the significant health disparities experienced by people affected by mental health and substance use disordersCascadia isseekinganinnovativediligentand compassionateInternal Medicine or Family provide patient care with team-based support at Cascadias outpatient clinics and Cascadias residential or community sites as directed by the Medical Director. TheMDwill be principally responsible for oversite of team-based primary care delivery ata CascadiaHealthCenter butwillalsoprovide patient carewith team-based supportatCascadiasotherclinicsresidential or communitysitesas directed by theMedical Director. This positionwillmeet regularly with theteam ofpeer primary care providers psychiatric providers andthe Primary Care Medical Directortohelpdesign implement and continuously improveour pioneering integratedprimary care of the care provided by theMDwill include Medication Supported Recovery (Suboxone) and psychiatricmedicationsmanagement for low acuity or stabilized position may sharelow volumeafterhourscall rotation (triaged througha nurse call line)with the other primary care providers.

Essential Responsibilities

  • Provide primary care diagnosis and treatment integrated within Cascadias behavioral healthcare services to clientswith mental health or substance use disordersand patients assigned by our Coordinated Care Organizations.

  • Lead the primary care staff inteam basedcare integrated with behavioral health staff at the assignedclinic working as a peer with psychiatric doctors and nurse practitioners at that clinic.

  • Provide primary care consultation to Cascadias psychiatric providers and behavioral health clinicians.

  • Provide medications management and Medication Supported Recoveryfor low acuity orstabilized individuals incoordination with their psychiatric provider.

  • Work with the integrated staff ata health centerand other assigned sitesin the development of wellness programs.

  • Share afterhourscall rotation (triaged through a nurse call line) with the other primary care providers.

  • Work withthe Primary Care Medical Directorand provider teamto define goals and review the progress of integrated primary care operations.

  • Regularly reviewand continuously improvequality of care provided by your clinic or care teamthrough periodic chart review qualitydashboards daily huddlesandrapid improvement cycles.

  • Work with the agency Clinic Directors and facility support staff in the spirit of stewardship to helpassurethe agency systems and facilities continuouslyoperateathigh levelsof efficacy and harmony.

  • Demonstrate responsibility forsafetyof consumers and/or residentsstaffand property;demonstratefamiliaritywith fire regulations and evacuation procedures.

  • Maintain a high level of fluencyin the useofthe electronichealthrecord.

  • Delivertimelyaccurate and professional documentation.

  • Adhere to mandatory abuse reporting laws and HIPAA requirements.

  • Provide input on all relevant documentation and procedural requirements.

  • Complete annual employee training requirements ona timelybasisperagency requirements. Attend seminarstrainingand other educational opportunities in order to develop professional skills and abilities.

  • Participate in all scheduled staff meetings supervision sessions and other departmental and agency meetings.

  • Perform other duties as assigned.

Qualifications

Education:

  • Requires an active state medical licensein AdultMedicine or Internal Medicine MD or DO including maintenance requirements and DEA certification.

Experience:

  • Threeyears of clinical experience with primary care.

  • Experience using an electronic health record (EPIC preferred).

  • Experience serving communities of color and immigrant populations preferred.

  • Experience with community healthclinic community mental health or homeless and special needs populations preferred.

  • Leadership experiencepreferred.

Specialized KnowledgeSkillsand Abilities:

  • Sufficient computer skills to complete documentation in an electronic medical records systemin a timely manner

  • Provide care in a setting consistent with the principles of collaborative care informed by stages of change models

Other:

  • Abilityto perform CPR First Aid andproperly useAED First Aid certifications arerequiredand must remain current.

Working Conditions

Work is performed in a typical clinical such the setting is not secure and the LMP willbe responsible forcreating a safe environment within the some cases safety personnel may be available toassistwith this care providerswill beprovidedtheappropriate tools with which to meet the requirements of their work (e.g.- computer printer scale medical equipment and supplies etc.).Meal periods and breaks are subject to interruption. Universal Precautions and remaining alert to the environment are critical to address the potential for violent behavior exposure to disease biohazardsnoiseand contaminants.

Mental Demands: TheMDmust be able to handle a variableworkflow manage time effectively prioritize activities and help primary care staff with whatever tasks are necessary toestablishthis new primary care capacity. The work assigned is diverse and involves addressing new and unusual circumstances in which outcomes may negatively affect costs employeemoraleor work regularly involves a degree of unpredictability and disruption of planned tasks requiring a flexible time management approach.

Physical Demands: TheMDmust have the ability toassistthe team in providing safety to all clients and staff in potentially volatile be able to escort clients between waiting areas and their meeting space before and after the encounter.
