Carolinas Rehabilitation Mt. Holly is seeking a dedicated Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Physical Medicine & Rehabilitation (PM&R) team at Carolinas Rehabilitation Mount Holly. This is a full-time position with a consistent MondayFriday schedule and no call responsibilities. The role is well suited for an APP interested in helping patients restore function improve mobility and return to daily activities through comprehensive rehabilitation care.

The Advanced Practice Provider will provide comprehensiveinpatient andoutpatient evaluation and management of patients with functional impairments related to musculoskeletal and neurological conditions. Responsibilities include performing patient assessments obtaining detailed histories conducting physical and functional exams and developing individualized treatment plans. The APP will order and interpretdiagnosticstudies prescribe medications and implement therapeutic role includes close collaboration witha robustinterdisciplinary teamto coordinate care and support patient progress toward functional goals. The APP willmonitorpatient outcomes adjust care plans andfacilitatereferrals across the continuum of rehabilitation services. Patient and family educationisa keycomponentof theposition with emphasis on diagnosis recovery expectations and strategies to support independence and long-term function.
Essential Functions
- Performs initial and periodic medical history physical examinations and daily assessments.
- Records findings of examinations assessments and plan of care.
- Performs minor office-based medical and/or surgical procedures.
- Writes appropriate prescriptions and administers medications.
- Orders and interprets daily laboratory and radiology examinations.
- Determines treatment plan for patient.
- Documents all patient encounters records observations interventions and actions in a medically appropriate manner on the patients medical record.
- Acts as a patient advocate for the population served.
- Attends participates and contributes to group/practice meetings and development of policies and procedures when requested.
Physical Requirements
Work requires walking standing sitting lifting reaching bending stooping pushing and pulling. Must be able to lift and support the weight of 35 pounds in handling patients medical equipment and supplies. Must speak English fluently and in understandable terms. Must possess intact sense of sight hearing smell touch and finger dexterity. Must have the ability to react and perform in stressful situations
Schedule
This position offers a MondayFriday schedule with no call requirements and is based at Carolinas Rehabilitation Mount Holly in Belmont North Carolina.
Qualifications
Qualified candidates must be graduates of an accredited Nurse Practitioner or Physician Assistant program and eligible for North Carolina mustpossessor be eligible for DEA licensure. Prior experience in physical medicine and rehabilitation orthopedics neurology or a related specialty ispreferredbut not communicationskills and the ability to work collaboratively in a team-based environment are team is tremendouslyhighperformingand we welcome candidates whoaspire to be part ofthisoutstanding team.
ACNP AGACNP AGNP PNP or PNP-AC board certification preferred based population served.
