Job overview:
As the registered Nurse (RN) you will oversee all nursingand related services and tasks for individuals in Supported Living CommunityProtection Supported Living and Bed-based Diversion Programs at Hope HumanServices. These include completing nursing tasks overseeing nursing supporttraining and coordinating delegation of DSP level medical support coordinationof supports and services with community providers/hospitals and overseeing thecompanys compliance with RCW WAC and DDCS policy/contract duties include completing nursing assessments managing medicaldatabases consulting with plan writers and representing HHS in policy/contractnegotiations as it relates to nursing services and supports
Job Duties/Responsibilities
- Hospital admission meetings/Consistent ER point of communication.
- Medical Documentation Review.
- AVS review screening and filing.
- Hospital discharge coordination and medical/safety protocol development.
- Provide medical advice and nursing discretion.
- Creating and monitoring medical protocols.
- Providing nursing tasks.
- Monitoring complex plans for new medications including psychoactive medication medical procedures and potential drug interactions.
- Monitoring the use and effectiveness of on-going medications.
- Medical observations for tasks that require nursing judgement.
- Consulting and providing staff with instructions for medication refusals and medication window extensions.
- Providing complex medical guidance to DDCS CRM strengthening relationships with guardians and providing medical oversight to the operations team.
- Monitor medical appointments post-appointment specialist coordination post-hospital PCP appointments and support in removing barriers to medical services.
- Compiling and organizing HIPPA compliant medical records to ensure medical diagnosis are well documented accurate and supported by previous medical documentation.
- Providing medical guidance for what level of medical attention is required (wound refusal etc.) to alleviate strain on hospital resources.
- Identify how medical diagnosis may be contributing to behavior. Nurse will provide guidance to the Positive Behavior Support plan writer. The FA/PBSP template asks to explain how the diagnosis may impact the persons level of function and impact on behavior; a nurse has more technical training on how a diagnosis may impact a person.
- Provide medical evaluation to rule out pain/medical concerns prior to FA/PBSP development.
- Medication Administration Record (MAR) management updates monthly approvals auditing.
- Nurse Delegation oversight and management.
- Maintain electronic health record log database and serve as primary contact point for medical providers.
Qualifications
- At least 21 years of age.
- Bachelors or Masters degree in a related field.
- Current active Registered Nursing (RN) license in Washington State.
- Maintain 12 hours of continuing education credits annually.
- Valid CPR and First Aid certification.
- Ability to pass and maintain Washington State criminal background clearance.
- Valid Washington State drivers license personal auto insurance and acceptable driving record.
- Flexibility to travel/attend in-person meetings as needed.
- Experience in home settings; experience supporting individuals with developmental disabilities or autism spectrum disorders preferred.
- Strong understanding of human behavior learning principles motivation behavioral assessment methodologies and data analysis with the ability to interpret trends and adjust strategies accordingly.
- Proficiency in Microsoft Office Suite and willingness to learn additional software for data collection workflows and documentation.
- Demonstrated professional skills including clear written and verbal communication; relationship-building with staff participants and families; ability to work independently and collaboratively; effective time and priority management in fast-paced environments; proactive problem-solving; recognition of emerging concerns; and meeting quality and productivity standards.
- Minimum one year of experience in social service preferably within the intellectual/developmental disabilities (I/DD) or DDA/DDCS field (preferred).
- Experience with Nurse Delegation (preferred).
