A Brief Overview

Assumes responsibility for daily coordination of the residents care including comprehensive assessments care conferences and written plans of care. The RN-MDS Coordinator oversees coordinates & assumes responsibility for resident care which includes but is not limited to admission discharge and transitions of care. The MDS Coordinator assures that quarterly annual & significant change observations & assessments are completed in a timely fashion for assigned group of Residents. Member acts as a resident advocate provides leadership organization and direction to the nursing department.

What you will do

  • Audits resident charts care plans and charting to ensure documentation supports the care provided and reimbursement level.
  • Completes and transmits MDS assessment and reports in accordance with regulatory requirements and facility policies and procedures.
  • Identifies and communicates need for significant change in status assessments.
  • Ensures compliance with guidelines for participation in Medicare Medicaid and other benefit programs.
  • Maintains a viable working relationship with all disciplines involved in the planning of resident care.
  • Completes Discharge Assessment and entry/re-entry tracking forms per Federal regulations.
  • Schedules MDS assessments per guidelines as assigned and maintains a monthly calendar.
  • Notifies members of the IDT of changes or additions to monthly Care Plan Meeting schedule for new admission change of condition or interim meetings.
  • Electronically submits MDS information. Reviews revises corrects and resubmits as appropriate;. Contributes to the Interdisciplinary care plan by communicating any resident issues that are pertinent to the plan of care.
  • Participates in facility QA initiatives.

Required Education License/Certification or Work Experience:

  • Registered Nurse (RN) - Board of Nursing An active license in the state of practice Upon Hire

Preferred Education License/Certification or Work Experience:

  • 1-3 years Long Term Care nursing with MDS experience
  • 1-3 years Geriatric or rehabilitation nursing
