• Conduct comprehensive speech language cognitive-communication and swallowing assessments
  
  
• Design and implement individualized treatment plans to improve communication swallowing and cognitive skills
  
  
• Provide hands-on therapy and practical interventions to promote progress and independence
  
  
• Educate patients and caregivers with clear guidance and techniques for at-home exercises
  
  
• Track and adjust therapy plans based on patient progress ensuring optimal outcomes
  
  
• Collaborate with an interdisciplinary care team (nurses physicians PTs OTs and more) to deliver coordinated care
  
  
• Maintain accurate timely documentation to meet clinical and compliance standards
  
  

• Masters degree in Speech-Language Pathology from an accredited program
  
  
• Active Virginia SLP license and CPR certification
  
  
• 2 years of experience as a Speech Therapist (home health experience preferred but not required)
  
  
• Strong clinical judgment and evidence-based practice skills
  
  
• Excellent communication and collaboration abilities
  
  
• Compassionate patient-first approach
  
  
• Experience with Home Care Home Base (HCHB) is a plus
