Join Wasatch Behavioral Health as a Full Time Case Manager at our Wasatch County Family Clinic in Heber City Utah where youll earn $23.35$32.68/hour while making a real difference in the lives of individuals and families navigating mental health and substance use challenges. Our clinic provides a wide range of therapeutic services in a collaborative team-based setting working alongside doctors nurses therapists prevention specialists and a family resource facilitator to deliver compassionate high-quality care to adults children and families in our community.
Schedule: MondayFriday 8:00 AM5:00 PM
Compensation: $23.35$32.68 DOE and Licensure

What Youll Do as a Case Manager
- Provide direct case management services to individuals experiencing mental health and/or substance use challenges
- Provide on-scene de-escalation safety assessments and immediate stabilization
- Complete assessments and develop individualized service plans based on client needs and goals
- Conduct crisis assessments and connect clients to urgent and ongoing services
- Coordinate care with therapists medical providers law enforcement EMS hospitals and community partners
- Support clients and families with safety planning crisis education and ongoing support
- Connect clients to housing employment financial assistance and other community resources
- Teach coping skills and independent living skills to support stability and recovery
- Advocate for clients in treatment team meetings community settings and court when needed
- Monitor client progress and adjust service plans as appropriate
- Maintain accurate timely documentation in accordance with agency and state guidelines
- Collaborate with a multidisciplinary team to provide holistic client-centered care
Qualifications
Applicants must meet ONE of the following:
Note: Applicants under the working toward SSW pathway must obtain their SSW license within 24 months of hire.
- Bachelors degree with Social Service Worker (SSW) license
- Bachelors degree in any field with willingness to pursue SSW licensure
Additional Requirements:
- Valid drivers license
- Basic understanding of Social Work principles
- Familiarity with social welfare policy and human development
- Ability to problem-solve and maintain a client-centered approach
- Spanish speakers may qualify for increased wages after testing
