The Role

Based on the Central Coast you will provide advanced assessment diagnosis prescribing treatment and care planning for residents within Residential Aged Care Homes. You will work within a collaborative multidisciplinary model alongside on-site nurses General Practitioners telehealth Geriatricians and Physicians with dedicated administrative and operational support so your focus remains on delivering high-quality clinical care.

What Youll Bring

• AHPRA registration as a Registered Nurse with Nurse Practitioner endorsement
• Experience in aged care gerontology palliative care or chronic disease management highly regarded
• Advanced clinical experience in assessment diagnosis treatment and health promotion
• Strong clinical reasoning communication and leadership skills
• Experience in quality improvement and evidence-based practice
• Sound knowledge of aged care legislation and regulatory requirements
• Current drivers licence reliable vehicle and professional indemnity insurance
• Comfortable using technology and clinical software systems

Qualifications

• Master of Nursing (Nurse Practitioner)
• Current AHPRA registration as a Registered Nurse with Nurse Practitioner endorsement
