We are currently seeking a Care Navigator to join our team and help us provide exceptional care to our clients. As a Care Navigator you will play a crucial role in coordinating and managing the care of our clients ensuring that their needs are met and their well-being is maintained. Key Responsibilities: - Conduct initial assessments of clients needs and develop care plans in collaboration with the client their family and other healthcare professionals - Coordinate and schedule appointments transportation and other services for clients - Monitor clients health and well-being including medication management and vital signs - Communicate with clients families and healthcare providers to provide updates and address any concerns - Provide support and guidance to clients and their families on managing chronic conditions and promoting healthy lifestyles - Maintain accurate and up-to-date documentation of clients care plans and progress - Participate in team meetings and collaborate with other healthcare professionals to ensure comprehensive and coordinated care for clients Qualifications: - Active LPN license in the state of Florida - Minimum of 2 years of experience in a healthcare setting preferably in home care or geriatrics - Excellent communication and interpersonal skills - Strong organizational and time-management skills - Ability to work independently and as part of a team - Proficiency in electronic medical records and other computer applications - Compassionate and patient-centered approach to care We offer competitive compensation and benefits as well as ongoing training and professional development opportunities.
