Your Role: Home Health LVN

As aHome Health Licensed Vocational Nurse (LVN) you will provideskilled nursing care to patients in their homesunder the supervision of a Registered Nurse (RN). This role is ideal for LVNs experienced or interested inhome health geriatric care dementia care and community-based nursing.

Primary Responsibilities

Provideskilled home health nursing carebased on individualized care plans
Deliver care togeriatric and senior patientsin home-based and assisted living settings
Accurately document visits treatments and assessments in the EMR system
Collaborate with physicians RNs families and interdisciplinary care teams
Support patients withchronic conditions recovery and dementia care needs
Ensure compliance withCalifornia home health regulations and company policies
Promote patient independence safety and overall well-being

Qualifications

ActiveCalifornia Licensed Vocational Nurse (LVN) licensein good standing
Graduate of an accredited vocational nursing program
Minimum1 year of nursing experience(Home Health experience preferred)
Experience withgeriatric patients senior care or dementia carepreferred
Valid drivers license reliable transportation and auto insurance
Strongcommunication time management and documentation skills
Comfortable using electronic medical records (EMR systems)
