Dove Healthcare West Eau Claire is seeking a Registered Nurse (RN) Case Manager to join our team. If youre ready to take the next step in your nursing career and use your experience to lead mentor and support a team while overseeing resident care this could be the perfect fit.

Schedule:

• Flexible business hours MondayFriday
• On-call every 6th weekend and every 6th weekday

What Youll Do:

• Coordinate and oversee care to ensure quality outcomes for residents
• Support palliative care services focused on comfort symptom management and quality of life
• Collaborate with rehabilitation teams to help residents reach their highest level of function
• Supervise day-to-day care and team member performance
• Ensure care plans are followed updated and compliant with regulations
• Assist with staffing coordination and team support
• Foster a safe comfortable and home-like environment for residents
• Participate in on-call rotation

What Were Looking For:

• Current Wisconsin RN license
• Graduate of an accredited nursing program
• Previous experience in a related setting
• Strong leadership organization and problem-solving skills
• Ability to manage priorities in a fast-paced environment
• Team-oriented with excellent communication skills
• Passion for providing high-quality compassionate care
