Key Responsibilities Certified Home Health Aide (CHHA)

• Provide personal care assistance including bathing grooming dressing and toileting
• Support mobility and rehab activities including transfers ambulation and exercises
• Monitor and record vital signs (temperature pulse respiration) during home visits
• Assist with meal preparation and feeding based on patient care plans
• Offer medication reminders (no administration required)
• Perform light housekeeping to maintain a safe and clean home environment
• Observe and report changes in patient condition to the clinical team
• Follow individualized home health care plans under RN or therapist supervision

Qualifications CHHA (Home Health)

• Active Certified Home Health Aide (CHHA) certification (California)
• CPR certification (current and valid)
• Reliable transportation and ability to travel throughout Los AngelesCounty
• Strong communication skills and commitment to patient-centered care
• Home health or community care experience preferred (not required)
