About the Role: Certified Home Health Aide (CHHA)

As aCHHA youll be providing essential personal care and support services in the comfort of your patients homes. Youll follow an individualized care plan under the direction of a registered nurse or therapist helping patients with daily tasks while preserving their dignity and independence.

Key Responsibilities:

• Personal Care Assistance:Help patients with bathing grooming dressing and toileting

• Mobility Support:Assist with transfers walking exercises and fall prevention

• Monitor Vital Signs:Check and document temperature pulse and respiration during each visit

• Meal Preparation:Assist with meals and feeding based on the patients dietary plan

• Medication Reminders:Prompt patients to take prescribed medications (no administration required)

• Light Housekeeping:Change bed linens keep the environment tidy and safe

• Observe & Report:Monitor patient condition and communicate changes to clinical staff

• Follow Care Plan:Deliver care based on each patients personalized home health care plan

Requirements:

• Current CHHA Certificationthrough the California Department of Public Health

• Reliable Transportation Youll be traveling to patients homes across Orange County

• CPR Certification Current and valid

• Strong Communication Skills Fluent in spoken and written English

• Empathy & Professionalism Compassionate respectful and dedicated to patient-centered care

• Home Health Experience Preferred but not required
