Registered Nurse- Quality Management (Transitional Care) Hybrid

POSITION SUMMARY:The Transitional Care Management Nurse utilizes a collaborative process of assessment planning facilitation and advocacy for options and services to meet Medicare patients health needs through communication and targeted intervention to promote optimal cost-effective outcomes in a clinic setting. The function of this position is to assist Medicare patients in the administration of their health plan benefits. The Transitional Care Management Nurse focuses on arranging or coordinating services that the patient needs to get well or stay well and remove barriers that prevent the member from engaging in an appropriate plan of care. The Transitional Care Management Nurse demonstrates strengths working independently as well as collaboratively within a multi-specialty healthcare environment. The Transitional Care Management Nurse performs interactive contact and certain non-face-to-face services with the clinics Medicare population that has been discharged from the hospital. The Transitional Care Management Nurse performs duties and responsibilities in a manner consistent with the Mission and Values of Hattiesburg Clinic. EDUCATION & EXPERIENCE:Graduation certificate from an accredited RN school of nursing requiredCurrent state licensure requiredThree (3) years of experience preferredIV certificationACLS certificationBLS/CPR certification preferredQUALIFICATIONS:Ability to maintain strict confidentiality and handle sensitive information with discretionAbility to maintain composure when confronted with fast-paced and stressful situations/timelinesAbility and desire to learn new technologiesMust be timely and dependableMust be able to multi-task work independently and be very detail orientedMust have excellent written and oral communication skillsMust be able to complete documentation accuratelyMust be able to follow existing workflowsKnowledge of EMR medical management and policies and proceduresIntermediate skills in work processing spreadsheet and database development and maintenanceWORK SCHEDULE:Monday-Friday 8:00AM-5:00PMHybrid after training is complete
