Hours Per Week: Per Diem
Schedule: 6:00 AM - 2:00 PM OR 2:00 PM - 10:00 PM 2 weekend shifts per schedule period
SUMMARY
Support Long Term Care staff and residents by providing residents with close direct observation or 1:1 supervision. Will work under the direction of a Registered or Licensed Practical Nurse. Observation of residents include those who are at risk of harming themselves or others if left unattended.
RESPONSIBILITIES
Follow the residents plan of care.
Observe for changes in condition and report findings to the appropriate nursing staff
Must be knowledgeable and adaptable to understanding and responding to the emotional and cognitive state of the resident.
Obtains verbal report from residents nurse and Interdisciplinary Care Team to include information regarding the residents identified care needs diagnosis visual auditory or language barriers as well as any special considerations. Obtains instruction from nurse or Interdisciplinary Care Team regarding ambulation limitations and course of action for redirection of resident if needed.
Maintaining documentation.
Maintaining confidentiality of patient information.
Adhering to relevant policies procedures and standards.
Maintains a safe and clean environment.
REQUIRED QUALIFICATIONS
High school diploma GED or suitable equivalent preferred
PREFERRED QUALIFICATIONS
Outstanding communication listening and observation skills.
Ability to respond quickly and appropriately.
Ability to work well together with others.
Professional appearance and proficient personal hygiene.
Undeterred by distressed or combative patients.
Available to work all shifts over weekends and holidays.

PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations including continuing education requirements.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience relevant qualifications specialty internal equity location and contracts.
