About the Role:
Join Hospice of Grace and Palliative Care Inc. as a Hospice Field LVN and make a meaningful impact in the lives of patients and their families. Responsibilities:
- Provide direct patient care in accordance with established care plans and protocols.
- Administer medications and treatments as prescribed by physicians.
- Monitor and document patients vital signs and health status.
- Assist with activities of daily living (ADLs) to promote patient comfort.
- Collaborate with interdisciplinary teams to enhance patient care.
- Educate patients and families on care plans and health management.
- Participate in on-call duties as needed for patient support.
- Maintain accurate and timely documentation of patient care activities.
Requirements:
- Current California LVN license with a strong understanding of palliative care principles.
- Minimum of 1 year of nursing experience preferably in hospice or home health care.
- Strong communication skills and the ability to empathize with patients and families.
- Ability to work independently and manage time effectively.
- Basic Life Support (BLS) certification required.
- Valid drivers license and reliable transportation for patient visits.
- Proficient in electronic medical records (EMR) systems.
- Compassionate demeanor with a commitment to quality patient care.

Compensation: $32.00 - $48.00 per hour
