Schedule: Every other weekend required Floating between hospital units and campuses
Position Overview
Navitas Healthcare LLC is seeking an experienced Licensed Master Social Worker (LMSW) with acute care background to join a hospital-based Case Management team. The role focuses on delivering comprehensive psychosocial support discharge planning and care coordination across multiple inpatient units and campuses.
Key Responsibilities
- Provide case management and care coordination for patients across multiple hospital units
- Conduct psychosocial assessments and develop effective discharge plans
- Coordinate safe and timely transitions of care
- Collaborate with interdisciplinary teams including physicians nurses and therapists
- Support patients and families with counseling and community resource linkage
- Maintain accurate and timely documentation in EPIC EHR system
- Float between campuses and units as operationally required (locations within close proximity)
- Participate in an every-other-weekend rotation (guaranteed 72 hours per two-week period)
Requirements
- Active Washington State LMSW license (required)
- Minimum 1 year of acute care social work experience
- Experience in discharge planning and care coordination
- Strong background in hospital-based case management (Med/Surg ED Long Length of Stay preferred)
- Current BLS certification required
- Ability to work across multiple units and campuses as needed
