WHO WE ARE

Since 1971 the Hampton-Newport News Community Services Board has provided a comprehensive continuum of high-quality essential and community-based services to citizens of the Peninsula who are living with mental illness developmental disabilities and substance use disorders.

Assertive Community Treatment or ACT is a team of medical behavioral health and rehabilitative professionals who use a team approach to meet the needs of an individual living with severe and persistent mental illness. Admission criteria include severe and persistent mental illness predominantly Schizophrenia and high service needs to maintain stability.

ACT consists of the following services: service coordination psychiatric assessment and treatment medication management substance use services counseling/therapy vocational services mental health support services peer support and representative payee services. ACT services are flexible and teams offer personalized levels of care for all individuals adjusting service levels to reflect needs as they change over time.

Goal of ACT: To lessen or eliminate debilitating symptoms of mental illness each client experiences and to minimize or prevent recurrent episodes of illness assist clients to meet basic needs and enhance their quality of life improve functioning in adult social and employment roles and activities increase stability in the community decrease hospitalizations and lessen the familys burden of providing care.

YOUR DAY TO DAY AS A SERVICE COORDINATOR

In this role you will work closely with clients families and care providers to assess needs determine eligibility for services and develop individualized treatment plans while regularly reviewing progress and updating goals. Your day will involve coordinating care responding to service inquiries and advocating for clients by connecting them with essential resources such as housing food healthcare and benefits. You will monitor client progress through meetings home visits and appointment scheduling support medication compliance and provide both ongoing and crisis counseling when needed. Responsibilities also include making referrals participating in team case reviews assisting with discharge planning and maintaining accurate documentation and reports. You may consult with colleagues on complex cases attend legal proceedings on behalf of clients and help maintain resource networks. Additionally you will conduct assessments such as HIV risk screenings coordinate care for clients with co-occurring conditions perform on-site drug screenings when required and carry out other related duties to support effective service delivery.

WHAT WE ARE LOOKING FOR IN A SERVICE COORDINATOR

To qualify for this position candidates must meet the following requirements:

  • Bachelors degree in Human Services such as Social Work Psychology or Counseling.
  • One (1) year of experience in case management or service coordination services demonstrating a solid foundation in addressing the needs of individuals with severe and persistent mental illness.
  • Must be registered with the Board of Counseling to practice as a QMHP or register with the Board of Counseling to practice as a QMHP trainee within the first 30 days of employment.
