UW Medicine Primary Care at Outpatient Medical Center has an outstanding opportunity for a Senior Social Worker.
WORK SCHEDULE
- 40 hours per week
- Monday-Friday
- Hybrid role- up to 2 days per week work from home after probationary period
- Day Shift
DEPARTMENT DESCRIPTION
Located just off of I-5 in north Seattle UW Medicine Primary Care at Outpatient Medical Center is open six days a week. (Our Saturday schedules vary. Please call ahead before visiting on a Saturday.)
Our board-certified physicians provide a full spectrum of primary care including annual wellness and preventive healthcare services management of chronic conditions treatment of acute conditions mental health services and routine gynecologic exams. We believe that continuity of care a shared goal that fosters ongoing caring relationships between patients and healthcare providers is vital to keeping you healthy in the long term.
POSITION HIGHLIGHTS
- Provides professional social work services in a client/patient clinical teaching and research environment.
- Provides social work services to clients/patients their families and/or others all while exercising independent decision-making authority and expertise specific to assigned area of practice.
PRIMARY JOB RESPONSIBILITIES
- Provides specialty consultations among health care staff and health care teams regarding psychosocially focused aspects of client/patient care.
- Provides psychosocial support and support utilization guidance to patients and families.
- Works in collaboration with a multidisciplinary team to develop and implement safe effective case management and counseling programs in specialized programs.
- Provides care coordination for patients with complex social and medical histories.
- Makes timely referrals to hospital and/community agencies including those required by mandatory reporting laws such and child and vulnerable adult abuse.
- Participates in practicum instruction of social work students and provides work direction to Social Work Assistant(s) and other staff.
- Engages in departmental program development activities tasks forces and committees.
REQUIREMENTS
- Masters of Social Work Degree from a program accredited by the Council on Social Work Education AND One year of full-time social work experience in a health care setting or equivalent. (Experience can include a practicum placement as part of a social work training program and/or employment experience.)
- WASHINGTON STATE LEGAL REQUIREMENT: Within 90 days of hire must hold one of the following credentials issued by the state of Washington:
- Agency Affiliated Counselor OR
- Licensed Social Work Associate - Advanced OR
- Licensed Social Work Associate - Independent Clinical OR
- Licensed Advance Social Worker OR
- Licensed Independent Clinical Social Worker
