Patient Care Manager, MDH Complex Transitional Care and Regional Unit

Responsible for the daily operations of the Complex Transitional Care and Regional Unit at Milton District Hospital. This position includes implementing and supporting clinical program priorities and innovations in patient care while ensuring a patient-centred approach quality patient care collaborative team work and fiscal responsibility.  Will have on-call administration duties for Milton District Hospital.

Qualifications :

• Health Care Professional
• Current certificate of registration and in good standing with the associated college
• Nursing () preferred
• Masters level preparation in business or health care field preferred
• 3-5 years leadership experience in acute care
• Proven leadership and change management skills
• Excellent written and verbal communication skills
• Proven financial management abilities
• Demonstrated ability to support team building staff development and conflict resolution

This position includes implementing and supporting clinical program priorities and innovations in patient care while ensuring a patient-centered approach quality patient care collaborative team work and fiscal responsibility.

Additional Information :

Hours: Monday to Friday 7.5 hour days plus leader oncall rotation

Site: Milton 

Salary: $66.122 to $82.639 per hour
