Description:
GENERAL SUMMARY AND PURPOSE:
Provide evidence based individual group family and educational services including assessment and psychotherapy. Document clinical and consultative activities in patient electronic medical record. Participates as a member of a multi-disciplinary treatment team to help assure the most appropriate treatment of patients. Works to develop and promote the programs and objectives of the department.
SKILLS KNOWLEDGE EDUCATION AND EXPERIENCE:
1. 1. Licensed Clinical Professional Counselor (LCPC) or Licensed Clinical Social Worker (LCSW) in the State of Idaho
2. One to three years of experience in a mental health setting required.
3. Experience in multiple therapy techniques preferred.
ESSENTIAL FUNCTIONS:
1. Meets Health Systems Core Values and Caring Standards including interpersonal communication and professional conduct expectations.
2. Demonstrates knowledge and skills to competently care for all assigned age groups (Child Adolescent Adult and Geriatric as applicable).
3. Conduct evaluation of patients and collaborate with healthcare team members to support the emotional mental environmental bio-psychosocial and spiritual needs of the patient and/or their family/caregivers as applicable.
4. Ability to clearly document at each visit objectives measurable goals and progress toward goals and identify the expectations of counseling with patients.
5. Ability to recognize when patient has achieved maximum benefit from therapy and professionally terminate care when patient is not progressing toward goals despite any needed treatment plan modifications or when patient has reached their goals.
6. Counsel patients and families regarding recommended levels of care community resources and financial arrangements as the program defines.
7. Interventions may include but are not limited to individual or family counseling referral to internal and external community resources crisis intervention grief counseling supportive therapy substance abuse referral support groups.
8. Complete all patient care documentation within the standards of the Trinity Health Timely Documentation policy.
9. Participates in team meetings.
10. Mentors students during internships as applicable.
11. Participates in community meetings as needed.
12. All other duties as assigned.
