Job Title: LPN / LVN (Licensed Practical Nurse / Licensed Vocational Nurse)

Assignment Type: Contract

Schedule:3x8s (3 shifts/week)

Weekend Requirement: Every Other Weekend

Job Summary:

We are seeking a dependable and experienced LPN/LVN for an assignment in a nursing home setting. The ideal candidate will provide quality patient care support the clinical team and ensure adherence to facility standards and protocols.

Key Responsibilities:

• Provide direct patient care and administer medications as prescribed
• Monitor patient conditions and promptly report any changes
• Assist in developing and implementing patient care plans
• Support CNA/PCA staff and assist with patient care duties as needed
• Maintain accurate and timely documentation using PointClickCare EMR
• Ensure patient safety comfort and well-being throughout the shift
• Follow all facility policies procedures and infection control guidelines

Required Qualifications:

• Active LPN/LVN License (required)
• 2 years of recent experience in a nursing home or long-term care setting (required)
• Hands-on experience with PointClickCare EMR (required)
• Willingness to assist with CNA duties when necessary
• Strong reliability punctuality and attendance record

Additional Requirements:

• No Requested Time Off (RTO) during the assignment
• Candidates must demonstrate consistent attendance (excessive tardiness or absences may result in early termination)
