The HSE has created six new health regions. Each region is responsible for providing both hospital and community care for the people in that area. Bringing community health services and hospitals together means we can take a more patient-centred approach to healthcare.
HSE Dublin North East provides health and social care to North Dublin Louth Meath Monaghan and most areas of Cavan.
Community Healthcare Services are delivered through the HSE and its funded agencies to people in local communities as close as possible to peoples homes. Community Healthcare Services focus on keeping you well so that you can continue to live at home or close to home through our health promotion disease screening diagnosis treatment and rehabilitation programs. You can refer yourself to most Community Healthcare Services or through your GP public health nurse community mental health team etc. Services include - Primary Care Social Care - Older Persons Social Care Disabilities Mental Health Health and Wellbeing Quality Safety and Service Improvement.

Community Occupational Therapy Services are provided on behalf of Primary Care and Social Primary Care Occupational Therapy services are provided to adults over 18 and Older People as part of the Primary Care Service.
Older Persons Services
Older Persons Services work in partnership with our colleagues across Integrated Health Areas of Dublin North County and Dublin North City & West and have expanded our collaboration and integration with the acute hospital system in recent years. Our aim is to ensure equality of access for the population of the Dublin IHAs to services support the acute hospital system to reduce Emergency Department presentations and hospital admissions through increased access to short stay rehabilitation and reablement services the specialist resources of the integrated care teams for older persons (ICPOP) and social support services.
Dublin North Integrated Care Team for Older Persons Community Specialist Team (ICPOP)
The DN ICPOP CSTs are based out of our new Integrated Care Hub in Omni Park Swords Road Santry Dublin 9. We are co-located with the Integrated Care Programme for Chronic Disease CST where we have both office & clinical space.
This is a community-based post where you will be working as part of a dynamic interdisciplinary team.
St Marys Campus
St Marys is a healthcare facility in the Phoenix Park that consists of 2 separate entities: St Marys Hospital and the Phoenix Park Community Nursing Unit (PPCNU).
St Marys Hospital:
In-patient services (101 beds) consist of rehabilitation of the older person both step down rehabilitation (i.e. transfers from the local acute hospitals) and step-up rehabilitation (i.e. community response admissions) and stroke rehabilitation for adults of all ages
In addition there is a Consultant Geriatrician led multi-disciplinary Day Hospital service provided from Monday to Friday
Phoenix Park Community Nursing Unit (PPCNU)
