Position Summary
The Medly Coordinator supports the delivery of high-quality care for heart failure patients by managing daily Medly alerts in collaboration with the Heart Function Nurse Practitioner and physicians. This role involves monitoring and triaging patient alerts communicating with patients and families to support proper use of Medly and providing education on heart failure management and self-care. The incumbent applies clinical knowledge and best practice guidelines to support guideline-directed medical therapy including medication titration while ensuring accurate documentation within the EPIC health information system in accordance with regulatory standards. Working closely with interdisciplinary teams referring providers and organizational partners the Medly Coordinator prioritizes workload effectively contributes to continuous quality improvement initiatives and promotes a collaborative and patient-centered care environment.
Duties
- Manages the daily (Monday-Friday) Medly alerts in partnership with the Heart Function Nurse Practitioner and Physicians
- Communicates with patients and families and reviews proper use of Medly; prioritizes alerts and escalating to the NP/MDs as appropriate.
- Provides patients and families with heart failure education on topics such as signs and symptoms of heart failure and self-care management of heart failure.
- Uses Medly to support Guideline-Directed Medical Therapy for heart failure and titration of medications.
- Liaise with a wide array of administrative and clinical team members throughout the organization and referring physicians and organizations.
- Prioritizes and triages daily workload to complete tasks within regular working hours.
- Documents actions and communication according to College of Nurses of Ontario standards using EPIC health information system.
- Applies best practice guidelines to ensure that patients receive care according to organizational and provincial standards.
- Contributes to the achievement of established department goals and objectives and adheres to department policies procedures quality standards and safety standards.
- Promote and maintain a positive and calming working relationship with team members and a healthy work environment.
- Participates in continuous quality improvement activities to enrich and advance processes through the effective use of resources and systems to deliver optimal patient care to our Indigenous health authority partners and community members.
- Performs cross-functional responsibilities and other duties consistent with the job classification as assigned.
Qualifications :
- Current registration with the College of Nurses of Ontario
- Bachelor of Science in Nursing; Masters preferred
- Current BCLS Certification
- Minimum Three (3) to Five (5) years Cardiology/C.V. Surgery Nursing and/or CICU/CVICU
- Strong organizational and time management skills
- Excellent interpersonal and customer service skills
- Excellent computer skills including Microsoft Office and EPIC applications
- Excellent verbal and written communication skills
- Knowledge and experience with heart failure guidelines and remote titration of HF patients an asset.
- Demonstrated ability to cultivate and build relationships
- Strong track record of high-quality work and engagement is essential
- Able to adapt to a constantly changing environment that requires ongoing revaluation of priorities
- Demonstrated ability to use good judgment in assessing difficult situations
- Able to work collaboratively within a group setting
- Proven track record of taking initiative
- Interest and commitment to health equity and to working in partnership with Indigenous peoples and communities to enable access to high-quality care.
- Completion of Cultural Safety and Principles of OCAP training are a definite asset. If not completed prior to hiring completion will be a requirement for the successful candidate
