We are seeking a licensed Physical Therapist (PT) to provide high-quality care in a home health setting. The ideal candidate will deliver patient-centered therapy services across designated service areas helping patients improve mobility restore function and achieve optimal independence in their homes.

• Evaluate and assess patients physical conditions and functional limitations
• Develop and implement individualized treatment plans
• Provide therapeutic exercises manual therapy and patient education
• Perform OASIS documentation and ensure timely accurate charting
• Monitor patient progress and adjust care plans as needed
• Communicate effectively with physicians caregivers and interdisciplinary teams
• Complete required documentation including visit notes and care coordination
• Travel to patient homes within assigned territories and provide care as scheduled

• Assigned coverage area within designated regions; travel outside primary area may be required
• Weekend coverage may be required based on patient needs
• Holiday coverage required

• Weekly productivity target equivalent to approximately 27 points per week
• Point-based system used to calculate workload and equivalent hours
• Visit types (including evaluations routine visits OASIS documentation recertifications and discharges) are assigned corresponding point values
• Clinicians may need to travel outside assigned areas or accept additional visits to meet productivity expectations

• Visits are tracked using a point-based system which translates into approved working hours
• Work includes patient visits documentation care coordination travel time and communication with patients and providers

• Active Physical Therapist (PT) license (state-specific)
• Minimum of 1 year of experience preferably in home health or a related setting
• Current BLS (Basic Life Support) certification
• Valid drivers license auto insurance and reliable transportation
• Strong clinical documentation and communication skills

• Current TB screening (annual)
• Immunizations: MMR Varicella Hepatitis B Tdap (within 10 years) and annual Influenza vaccine
• Pre-employment drug screening and physical exam required

• Must be willing to treat a variety of patient conditions including infectious cases as appropriate
• All time-off requests must be disclosed prior to assignment confirmation
• Flexibility and willingness to adapt to patient volume and scheduling needs are essential
