We are seeking a licensed Physical Therapist (PT) to provide high-quality care in a home health setting. The ideal candidate will deliver patient-centered therapy services across designated service areas helping patients improve mobility restore function and achieve optimal independence in their homes.
Key Responsibilities- Evaluate and assess patients physical conditions and functional limitations
- Develop and implement individualized treatment plans
- Provide therapeutic exercises manual therapy and patient education
- Perform OASIS documentation and ensure timely accurate charting
- Monitor patient progress and adjust care plans as needed
- Communicate effectively with physicians caregivers and interdisciplinary teams
- Complete required documentation including visit notes and care coordination
- Travel to patient homes within assigned territories and provide care as scheduled
- Assigned coverage area within designated regions; travel outside primary area may be required
- Weekend coverage may be required based on patient needs
- Holiday coverage required
- Weekly productivity target equivalent to approximately 27 points per week
- Point-based system used to calculate workload and equivalent hours
- Visit types (including evaluations routine visits OASIS documentation recertifications and discharges) are assigned corresponding point values
- Clinicians may need to travel outside assigned areas or accept additional visits to meet productivity expectations
- Visits are tracked using a point-based system which translates into approved working hours
- Work includes patient visits documentation care coordination travel time and communication with patients and providers
- Active Physical Therapist (PT) license (state-specific)
- Minimum of 1 year of experience preferably in home health or a related setting
- Current BLS (Basic Life Support) certification
- Valid drivers license auto insurance and reliable transportation
- Strong clinical documentation and communication skills
- Current TB screening (annual)
- Immunizations: MMR Varicella Hepatitis B Tdap (within 10 years) and annual Influenza vaccine
- Pre-employment drug screening and physical exam required
- Must be willing to treat a variety of patient conditions including infectious cases as appropriate
- All time-off requests must be disclosed prior to assignment confirmation
- Flexibility and willingness to adapt to patient volume and scheduling needs are essential
