We are seeking a dedicated Occupational Therapist (OT) to provide patient-centered care in a home health setting. This role focuses on helping patients regain independence in daily living activities through personalized therapeutic interventions. New graduates are welcome to apply and prior home health experience is preferred but not required.
Key Responsibilities- Evaluate patients functional abilities and develop individualized treatment plans
- Provide therapeutic interventions to improve daily living and occupational performance
- Conduct home safety assessments and recommend adaptive equipment or modifications
- Complete OASIS documentation and maintain accurate timely patient records
- Monitor patient progress and adjust care plans accordingly
- Collaborate with physicians caregivers and interdisciplinary teams
- Educate patients and families on treatment plans and self-care strategies
- Travel to patient homes within assigned service areas
- Coverage within designated service areas; travel outside primary territory may be required
- Weekend and holiday coverage may be required based on patient needs
- Weekly productivity target equivalent to approximately 27 points per week
- Point-based system used to calculate workload and corresponding hours
- Visit types (evaluations routine visits OASIS documentation recertifications etc.) are assigned point values
- Flexibility required to meet visit expectations including travel outside assigned areas if needed
- Work is tracked using a per-visit point-based system translating into approved hours
- Coverage includes patient visits travel time documentation and care coordination activities
- Active Occupational Therapist (OT) license (state-specific)
- Minimum of 1 year of experience preferred; new graduates welcome
- Current BLS (Basic Life Support) certification
- Valid drivers license auto insurance and reliable transportation
- Strong communication organizational and clinical skills
- Current TB screening (annual)
- Immunizations: MMR Varicella Hepatitis B Tdap (within 10 years) and annual Influenza vaccine
- Pre-employment drug screening and physical exam required
- Must be willing to treat a variety of patient conditions including infectious cases as appropriate
- Flexibility to accommodate caseload demands and scheduling needs is essential
- All requested time off must be disclosed prior to assignment confirmation
