We are seeking a licensed Physical Therapist (PT) to deliver high-quality patient-centered care in a home health setting. The ideal candidate will help patients improve mobility restore function and achieve independence through individualized treatment plans in their home environments.
Key Responsibilities- Perform comprehensive patient evaluations and develop individualized care plans
- Provide therapeutic interventions including exercises manual therapy and mobility training
- Complete OASIS documentation and maintain accurate timely records
- Monitor patient progress and adjust treatment plans as needed
- Educate patients and caregivers on treatment goals and home exercise programs
- Collaborate with physicians and interdisciplinary healthcare teams
- Travel to patient homes within assigned territories to deliver care
- Coverage within designated service areas; may include multiple territories
- Travel outside assigned areas may be required based on patient needs
- Weekend and holiday coverage may be required
- Weekly productivity target equivalent to approximately 27 points per week
- Point-based system used to determine workload and corresponding hours
- Visit types (evaluations routine visits OASIS documentation recertifications discharges etc.) are assigned specific point values
- Clinicians may need to accept additional visits or travel outside primary areas to meet productivity goals
- Work is measured using a per-visit point-based system that converts to approved hours
- Coverage includes patient visits documentation travel time and care coordination activities
- Active Physical Therapist (PT) license (state-specific)
- Minimum of 1 year of experience preferred; home health experience is a plus
- Current BLS (Basic Life Support) certification
- Valid drivers license auto insurance and reliable transportation
- Strong clinical communication and organizational skills
- Current TB screening (annual)
- Immunizations: MMR Varicella Hepatitis B Tdap (within 10 years) and annual Influenza vaccine
- Pre-employment drug screening and physical exam required
- Must be willing to treat a variety of patient conditions including infectious cases as appropriate
- Flexibility to accommodate caseload demands including travel and scheduling adjustments
- All requested time off must be disclosed prior to assignment confirmation
