ESSENTIAL FUNCTIONS

• Conforms to the National Association of Social Workers (NASW) Code of Ethics
• Assists patients and families with psychosocial needs including counseling as needed and coordination of family meetings
• Recognizes problems systematically gathers data identifies/understands underlying issues synthesizes complex issues seeks input from others and makes difficult decisions to formulate appropriate treatment plans.
• Engages and collaborates with patients families multi-disciplinary staff and the community in care planning delivery and discharge. Builds solid relationships negotiates effectively and works well with individuals of all ages backgrounds and socioeconomic position.
• Provides on-call services for Hospital Physician clinics and inpatient units.
• Advocates for patients and helps them navigate the health care delivery system.
• Functions as the liaison between patient physician adoptive agency or private attorney and patient accounts and as a support person for the mother placing her baby for adoption both prior to and following delivery.

MINIMUM REQUIREMENTS:

• Bachelor in Social Work required Masters in Social Work preferred
• Current Iowa License preferred
• Experience: a. One to three years professional social work experience preferred. Previous experience in acute care preferred.
