Medical Social Worker I position accepts the responsibility and accountability for addressing psychosocial financial emotional and environmental factors as they relate to patient health and discharge planning needs. This includes but is not limited to Advance Directive issues patient/family advocacy and coordination of community resources while maintaining professional social work standards.ESSENTIAL FUNCTIONS
- Conforms to the National Association of Social Workers (NASW) Code of Ethics
- Assists patients and families with psychosocial needs including counseling as needed and coordination of family meetings
- Recognizes problems systematically gathers data identifies/understands underlying issues synthesizes complex issues seeks input from others and makes difficult decisions to formulate appropriate treatment plans.
- Engages and collaborates with patients families multi-disciplinary staff and the community in care planning delivery and discharge. Builds solid relationships negotiates effectively and works well with individuals of all ages backgrounds and socioeconomic position.
- Provides on-call services for Hospital Physician clinics and inpatient units.
- Advocates for patients and helps them navigate the health care delivery system.
- Functions as the liaison between patient physician adoptive agency or private attorney and patient accounts and as a support person for the mother placing her baby for adoption both prior to and following delivery.
MINIMUM REQUIREMENTS:
- Bachelor in Social Work required Masters in Social Work preferred
- Current Iowa License preferred
- Experience: a. One to three years professional social work experience preferred. Previous experience in acute care preferred.
